Intrax Training Check-In Form
Remember: YOU MUST NOT CHECK-IN BEFORE YOU ARRIVE IN THE USA! If you do so, you will not be allowed to enter the country by the immigration authorities.
Instructions
Rules governing the Intrax Training Program require that you check-in with Intrax Training directly following your arrival in order to:
- Announce your arrival and living address
- Activate your health insurance
- Active your record in the Student and Exchange Visitor System (SEVIS)
Failure to complete the Check-In process will result in dismissal from the Training Program, no activation of your health insurance, termination of your program in the SEVIS system and an immediate return home!
In order to successfully complete your Check-In you must complete ALL of the following fields accurately.
